Any of the following will be accepted:
Social Security Verification Documents
- Photocopy of your Social Security Card
- Photocopy of your pay stub showing your full SSN
- Photocopy of your W-2
Any submission must display your FULL 9- digit Social Security Number
Address Verification Documents
- Photocopy of a pre-printed bill (utility, credit card, etc)
- Photocopy of your valid Drivers License and/or Identification card
Your Address Verification must display your name, alongside your CURRENT mailing address. If your driver's license has a previous address, you must use another document.
Please Note: If you are moving during your service, do not forget to update your address with Fresh Credit by phone or online at www.nationalcd.com
I authorize NCD to process this application to assist with the restoration and repair of all 3 credit profiles -Equifax, TransUnion, and Experian. I may request my credit reports at credithero.com. NCD DO NOT require you to maintain ANY monthly services. It is recommended that you download your 3 - in- 1 credit report and submit to: NCDCOLLECTIONS@OUTLOOK.COM. NCD will act as the Duly Appointed Agent while performing the requested services. Processing can take 60-90 days for the credit agencies to respond to discrepancies in accounts listed on the credit files. I understand that I will be REQUIRED to attend at least (1) credit education class. Class Schedules are announced monthly. To schedule a session call (702) 344-3317 Monday - Friday between the hours of 1 pm-3 pm PST. There is a non-refundable fee in the amount of $375.00 which is due before services can begin. I attest that the information provided on the application is true and correct.
Suppose you fail to disclose ANY discrepencies, error, prior identity theft, ID or Social Security (Spelling Errors), etc. You release Fresh Credit Program of ALL liability and waive your right to initiate a refund claim and no REFUND will be allowed. No Exceptions. If you are instructed to make corrections on your discrepncies, you are RESPONSIBLE to cure ANY ERRORS before the DEADLINE date provided to you in the email notification send by: NCDCOLLECTIONS@OUTLOOK.COM.
